Intake Process for Individuals

If you think the Felton Early Psychosis Programs might be the right place for you or someone you know, call us and we’ll help guide you through a simple assessment process.

Step 1 – Call your local Felton Early Psychosis Programs location. When you call, you can complete phone screening with one of our Felton Early Psychosis Programs staff members.

Step 2 – If eligible after the phone screen a Client and/or Family will complete an in-person comprehensive assessment with a Felton Early Psychosis Programs Clinician.

Step 3 – If after the assessment, a Client is found not to be eligible for Felton Early Psychosis Programs, we will invite the Client and/or Family to attend a feedback session where we will provide appropriate education, recommendations and referrals as needed.

Step 4 – If the Client does meets eligibility criteria for Felton Early Psychosis Programs, they’re admitted and a Plan of Care is developed collaboratively with the Client and/or Family, may include any or all aspects of treatment at Felton Early Psychosis Programs, plus collaboration with other providers.

Clients will remain eligible for Felton Early Psychosis Program’s services up to two (2) years from admittance.

Intake Process for Clinicians

Step 1 – Call your local Felton Early Psychosis Programs location and complete a telephone screening with of our Felton Early Psychosis Programs staff members.

Step 2 – Discuss referral with Client and/or Family and…

Step 3 – Schedule an appointment with Felton Early Psychosis Programs staff to meet one-on-one with Client and/or Family or have them call Felton Early Psychosis Programs to complete phone screen.

Step 4 – Client and/or Family will complete an in-person diagnostic assessment.

Step 5 – Felton Early Psychosis Programs staff will request discharge summary or other records and contact providers for collateral.

Step 6 – Client and/or Family attend feedback session with education, recommendations and referrals if not appropriate for Felton Early Psychosis Programs.

Step 7 – Plan of care developed collaboratively with Client and/or Family, may include any or all aspects of treatment at Felton Early Psychosis Programs, plus collaboration with other providers.

Clients will remain eligible for Felton Early Psychosis Program’s services up to two (2) years from admittance.
